This from must be completed and signed by the dancer's parent/guardian. Return it to Studio no later than Jan.31st or s/he will not be allowed to participate.

Please read becarefully and tick to agree below:

I/We the parent(s) or legal guardian(s) of the above-named dancer, consent to said participation with the Academy in rehearsals and performances of the " 2024 Chu’s Dance Recital". I/We assume all risks and hazards incidental to such participation including transportation to and from rehearsal, photo studio and performances. I/We hereby waive, release, absolve, indemnity and agree to hold harmless the academy, the organizers, Artistic Directors, Board Members, instructors, volunteers and their agents from any claim arising out of an injury to my/our child for any cause except to the extent and in the amount covered by accident or liability insurance.

LATE PICK UP FEE: I/We agree that my/our child will arrive at rehearsals and performances on the time and that my/our child is picked up directly at the end of the rehearsal/performance period. I/We understand that if my/our child is not pick up promptly at the designated times, I/we will be responsible to pay an amount of $30 minimum for each 15 minute increment.

I/ We agree that my/our child will release their video and photo's to the academy, organizers, Artistic Directors, Board Members, instructor and photographer, for use.

I/We understand that my/our child will not miss any scheduled rehearsals or performances. Should my/Our child miss a rehearsal or performance, I understand that no return application fee and s/he may be replaces by his/her understand for that scheduled rehearsal or performance and possibly for future rehearsals and performances.

CLOSED DOOR POLICY: We have a CLOSED DOOR POLICY at ALL REHEARSAL. It is our experience that students are better focused without the distraction of parents, siblings, and friends.

Add your textPERFORMANCES: All students are required to participate in school recital and annual performance. There are additional application fees charged. From junior level and up we required student to purchase costume for presenting a professional look on stage. Costumes can vary between $200-$500 depending on how elaborate you would like to go.

COSTUME RENTAL: All costume has a rental fee charge. For student rentals, absolutely NO dyeing, distressing, or permanent alterations are allowed. No major alterations may be done without the authorization of the instructor. All items must be returned clean and in good repair. Any costumes returned with damages will be assessed at full costume replacement value.

FUTURE POLICY CHANGES: Chu’s Dance Studio reserves the right to change the above policies at any time to reflect the needs and concerns of the studio. Student will be informed of changes to policies.
